MEDICAL REPORT ON: SHEIKH AHMED HOOSEN DEEDAT
HISTORY :Sheikh Ahmed Deedat developed a cerebral vascular accident resulting in the locked in syndrome about 6 years ago.He is a known diabetic for over 10 years.Previously he had been on diet alone.Subsequently he was on regime of humulin 30/70.He is currently at home being nursed by his wife.He obtains his nutrition via a gastrotomy.
EXAMINATION:He was conscious and appeared quite alert.His general condition was satisfactory.Pulse –76,regular,good volume.All his peripheral pulses including the carotid pulses were palpable.Blood pressure – 128/78.Cardio vascular system – he was not in cardiac failure.There was no clinical cardiomegaly.Heart sounds – normal.Chest – clear.Abdomen –he had a gastrotomy tube through which he was fed.Otherwise no abnormality was noted. Neurological assessment – he was consciuos and alert.There was no neck stiffness.He was unable to speak because of an arthria.He had a spastic quadriplegia with a plantar flexion deformity of both his feet (result of the previous CVA).Plantars were upgoing.Sensation,cranial nerve function-normal.
SPECIAL INVESTIGATIONS : Random blood glucose-5.6.Glycated haemoglobin-4.7 %-indicative of excellent diabetes control over the preceeding 2 to 3 mnths.U and E –no significant abnormality.Total cholesterol-3.07 with a LDL cholesterol of 1.21,HDL cholesterol of 1.02 and a triglyceride of 1.75 – all within the target range.He had a mild microcytic hypochromic anaemia of 11.7 with a MCV of 63.Platelet count and white cell count – normal.
COMMENT :Sheikh Ahmed Deedat has the locked in syndrome because of a cerebral vascular accident affecting the brain stem.He is fully alert and is to communicate via a chart whereby each word is formed by him looking at the letters of the alphabet and in turn he nods his head with the appropriate word.In this way he is able to communicate with complete sentences.His wife has looked after him remarkably well considering the fact he has no bed sors whatsoever.His current medicatoin comprises a twice daily regimen of humulin 30/70 as well as daily heparin.He requires ferrimed 1 daily for his iron deficiency anaemia.
